Each hospital has a core of social workers that are familiar with our program as well as other resources that could prove helpful. To find out if you would qualify for non-medical financial assistance from the Compassionate Touch™ Program, call the Social Service department in the hospital at which you or your loved one will be a patient.

IS THERE A FORM TO FILL OUT?
Compassionate Touch applications are located in most of the hospital’s Social Services Department. The patient or their family member/caregiver may contact their social worker to fill out an application for funding through the Compassionate Touch program. If the patient is in a hospital room, he/she may contact the nurse’s station and request that their social worker be paged to their hospital room.

WHAT ARE THE STEPS FOR THE PROCESSING OF THE APPLICATION?

Once the application is filled out completely, the social worker will fax the application to the Compassionate Touch office via a securely encrypted website to ensure patient confidentiality.
· All applications are processed according to the date and time received.
· The application will be processed and awarded within a 72 hour work-day turnaround upon the availability of funds. The  Compassionate Touch office is closed on weekends and holidays.
· Compassionate Touch personnel will contact the hospital social worker     concerning funding decision.
· The social worker will notify the patient / family member or caregiver.
